2025-2026 Club Sports Forms
CLUB SPORT STUDENT-ATHLETES:
All forms are to be returned to either your Head Coach, the Office of Club Sports or the Coordinator of Club & Intramural Sports. Any Club Sport Student-Athlete who does not return ALL of their forms, fully completed and signed/stamped by necessary doctors/medical professionals, before the posted deadline WILL NOT be permitted to practice and/or compete until the forms are returned. THERE WILL BE NO EXCEPTIONS!
PLEASE NOTE: that your personal physician MUST fill out/sign & stamp the PHYSICAL EXAMINATION FORM, the ADHD STIMULANT MEDICATION FORM and the SICKLE CELL TRAIT FORM provided by the Office of Club Sports. We cannot accept incomplete, unsigned, unstamped or any generic doctor's office form(s).

Form B - Physical Examination Form (Completed & Signed/Stamped by Personal Physician)
Form C - Medical History Questionnaire (Completed by All Club Sport Athletes)
Form D - HIPAA Statement (Completed by All Club Sport Athletes)
Form E - ADHD Stimulant Medication Form (Completed & Signed/Stamped by Personal Physician)
Form F - Sickle Cell Trait Form (Completed & Signed/Stamped by Personal Physician) & (For New or Transfer Club Sport Athletes Only)
Club Sports Athlete Conduct Agreement (Completed by All Club Sport Athletes)
Club Sports Waiver Form (Completed by All Club Sport Athletes)
Club Sports Participant Fee Form (Completed by All Club Sport Athletes)
