Student-Athletes are expected to read through the NCAA/STAC policies - there is no need to print
NEW/TRANSFER PHYSICAL FORM - to be completed by your personal physician
ASTHMA FORM - fill out with your personal physician and get doctor to stamp 2nd page yearly
NCAA ADHD MEDICATION POLICY - please call to discuss what is expected and needed yearly
